Simple Form FAQs

Q. How does it work?
A. Add a new event record, then start the kiosk to begin collecting responses. The app will present a series of brief entry forms to collect a response list. Each entry form has instructions to enter up to ten custom fields.

The resulting response records can be managed on the device, and exported to a comma-separated value file to be used externally.

Q. How do I customize a field?
A. Customize fields by tapping the Fields button. You can include up to ten response entry fields, and define each field type and placeholder text. Changes will apply to all events for a single form.

You can think of 'forms' as a way to organize different sets of entry fields and events. Forms can be named whatever is appropriate and it's easy to switch among them for different types of events.

Q. Can each form have different entry fields?
A. Response entry fields are configured per form. Each form has a separate set of fields.

You can think of 'forms' as a way to organize different sets of entry fields and events. Forms can be named whatever is appropriate and it's easy to switch among them for different types of events.

Q. Can I use my company's graphics in the app?
A. You can substitute your own images for the background and entry forms. Separate background images for portrait and landscape orientations are supported to allow optimal visibility of your company's graphics during kiosk response collection, especially while the keyboard is present. You can select the images from the device's photo album or from iCloud Drive. Go to Customize Kiosk, then select Custom Images.

You can also customize the appearance of the text that appears on the kiosk to match your custom images.

Q. How do I get the response records off the device?
A. The event and response records can be exported by the following methods to a CSV (comma-separated value) file format:

Email the file as an attachment

Open the file using other apps installed on the device that support CSV text files

Access the file in iTunes when connected to a computer

Access the file in iCloud Drive by logging into iCloud using your AppleID

Q. Can I keep the respondents from switching to other apps on my device?
A. Simple Form has an optional feature to control exit from the Kiosk Entry process.

In addition, you can use the iOS Guided Access feature that restricts use to a single app.

Visit the support website for detailed instructions on how to use Guided Access.

Q. How can I scan a QR or barcode into a kiosk entry field?
A. Select the Fields button, and then select any field you wish to configure for barcode support. Turn ON the Barcode Assisted switch. This will cause a button to appear on the fields you have configured for barcode support when you launch the Kiosk. Tapping the button will open the barcode scanner. Position the barcode near the device's rear camera so that it is centered on screen and parallel to the camera.

Once the barcode is captured, the entry field will be filled in with the data and control returned to the kiosk. You can still manually edit a field that is barcode assisted with the keyboard.

The following barcodes are currently supported: QR, PDF 417, UPC-E, Aztec, Code 39, Code 39 Mod 43, EAN-8, EAN-13, Code 93, and Code 128.

Q. How can I remotely monitor kiosk collection over wi-fi or bluetooth?
A. You can use Simple Form for iPhone to monitor kiosk response collection over a wi-fi or bluetooth connection on a nearby device that is also running the Simple Form app. The monitored kiosk can be running Simple Form for iPad or iPhone.

On the device that you want to monitor, run Simple Form and select the event you want to use for collecting responses. Then tap the Kiosk button and turn ON the option "Allow Remote Monitoring" before starting the kiosk. Make sure wi-fi or bluetooth is enabled on both devices.

On the device you want to use as monitor, run Simple Form and tap the Monitor button. An alert will appear on the monitored kiosk asking you to accept the invitation from the monitoring device. Any existing response records will be transferred to the monitoring device, and subsequent entries will transfer as they are entered.

You can optionally save the transferred response records on the monitoring device. Make sure the form's entry fields are configured the same on both devices if you want to save the monitored entries.

You can only connect one Monitor at a time to one remote Kiosk. Once a Form has connected a Monitor to a remote Kiosk, that Form is the only one that can reconnect to that particular Kiosk session. To connect a different Form or device to a remote Kiosk, the kiosk session must be restarted.

It is recommended that both the Kiosk and Monitor devices be set to Auto-Lock = Never in Settings > General. If either device auto-locks, iOS will disconnect the monitor session. When the locked device is unlocked, the monitor session will attempt to automatically reconnect, but there may be delays in doing so.

Q. Can I transfer existing response records to another device over wi-fi or bluetooth?
A. You can use Simple Form for iPhone to transfer response entries over a wi-fi or bluetooth connection from a nearby device that is also running the Simple Form app.

On the device with the records you want to transfer, run Simple Form and select the event with the records you want to transfer. Then tap the Kiosk button and turn ON the option "Allow Remote Monitoring" before starting the kiosk. Make sure wi-fi or bluetooth is enabled on both devices.

On the device you want to transfer the records to, run Simple Form and select the Monitor button. An alert will appear on the monitored kiosk asking you to accept the invitation from the other device. Existing response records will be transferred to the monitoring device, and any subsequent entries will transfer as they are entered.

You can save the transferred response records on the monitoring device. Make sure the form's entry fields are configured the same on both devices if you want to save the transferred entries.

Q. Does Simple Form support other languages?
A. The app is English-only, but custom text in any language supported by iOS can be substituted for text presented to respondents during the entry process, including placeholder text, entry instructions, and screensaver exit instructions.

Q. Why is the app not rotating when I turn my iPad?
A. Check your system settings to be sure you have Lock Rotation enabled for the side switch on the the device.
